Field Trip Requests and Student Lists (Staff Only)
ECVHS Field Trip request
Everyone requesting to go on a field trip will follow all the steps on the form below. ( You must be signed in to your staff GUHSD Google Account )
1. Complete this form at least two weeks prior to your field trip.
2. Upon completion of this form, immediately send an electronic list that includes student names, grade level and ID numbers to Juliana Snow.
3. You will receive notice within 48 hours of approval/disapproval along with student excursion permits.
Funding source is a critical component of a field trip. Funding includes travel costs, substitute costs for all staff attending and any fees associated with your activity. It is highly recommended you discuss field trip funding with administration or the MSF prior to filling out this request.
